What is the New Horizons Dementia and Behavioral Health Memory Care Program?

New Horizons Dementia and Behavioral Health Memory Care Program is a new Washington State contracted long term care program for those who are living with the commodities of dementia and behavioral/mental health concerns. New Horizons is a compassionate approach to memory care for those who have chronic mental or behavioral health concerns, as well as a form of dementia related illness. New Horizons is proud to be one of the few behavioral health memory care communities in the state.

We take an advanced services approach with our behavioral health memory care services. Here's how: 

  • Residents have their own personal space as well as being integrated into the social life of our New Horizon’s neighborhood.

  • Increased the intimate caregiver-to-resident ratio, typically 1 resident to 4 staff members.

  • Specialized and individual behavioral health service plans with focus on their leisure preferences, social histories, individual needs, and activity preferences.

  • Psychoactive medication management under the supervision of skilled psychiatric practitioners and professionals with bi-monthly reviews.

  • Weekly Social Work case management with a dedicated staff member.

  • Specialized recreational programming by experienced activities professionals.

Program requirements:

  • If DSHS-funded: The individual must be on a 90 day involuntary hold, have a dementia diagnosis per ICD-10 coding with DSHS Cognitive Performance Score of 3, and is DSHS approved for this unique program.
  • If private-pay funded: Rates start at $385 per day. The individual must have a dementia diagnosis that is advanced to warrant a secured setting.
